If you are going to travel abroad and think that you might need vaccinations you will need to complete our form. You can print this off by clicking into the link below or you can pick up a copy at the surgery. Leave the completed form at reception and allow a week for the nurses to check the vaccination schedules. You can then contact us to find out what vaccinations you need and make an appointment with the practice nurse. Ideally this should be 6-8 weeks before you travel because some courses of vaccinations take a while to complete.Please note that although some vaccinations are available on the NHS others have to be prescribed privately, and these will incur a prescription charge at the surgery as well as payment at the pharmacy.
