Online Services

stroudvalleys.familypracticetext@nhs.net

You are welcome to register for online services at the practice.  This will allow you to request your regular repeat medication, book or cancel appointments with the doctors and view your summary care medical record which includes your current medication as well as any sensitivites or allergies.

If you would like to do this, please come in to the surgery bringing with you some clear form of photographic identification - preferably a passport.  The reception staff will then be able to give you a user ID and password.

From April 2016 we will be able to offer access to detailed information that is held in coded form within a patient's medical records.  This would be from the date of registration for this service.  If you decide after April that you want to do this, please check with the surgery as we will require completion of a specific registration form and will need to recheck your identification
