New Patient Registration
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. For guidance the area covers all GL5 and many GL6 and GL10 postcodes.
You will need to complete registration forms which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You can print off the forms from the links below so that you can complete them and bring them to the surgery at your convenience.

Joining Up Your Information Consent
You will also need to bring with you proof of identity. This should ideally be photographic, preferably your passport or photo driving licence. If you don't have either of these please bring a recent utility bill, tax letter or similar addressed to you at your current local address. If you have recently entered the country please also bring your immigration papers if appropriate.
All new patients are requested to complete our registration questionnaire and Joining Up Your Information Consent form (please check links above). If you are aged between 45 and 75 you may be offered an appointment with our Health Care Assistant for a NHS health check. Of course you are welcome to make an appointment for a general review whatever your age.
